Diastolic Mitral Regurgitation in Acute Severe Aortic Regurgitation

Abstract

|
Diastolic mitral regurgitation (MR) in the absence of AV block may occur secondary to significant elevation of left ventricular end-diastolic filling pressures in the presence of
restrictive physiology or severe aortic regurgitation, primarily acute regurgitation. A 63-year-old man presented with sudden onset of shortness of breath and underwent transthoracic echocardiography for the evaluation of heart failure. The echocardiographic examination demonstrated severe aortic regurgitation with suspicious vegetations attached at ventricular side of the aortic valve. Color flow imaging revealed severe degree of aortic regurgitation. Pulsed-wave Doppler interrogation of mitral inflow revealed tall E velocity with shortened deceleration time suggesting restrictive physiology.
Diastolic MR was detected by multiple Doppler modalities(Fig. 1). Follow-up echocardiogram after medical treatment showed disappearance of diastolic MR by color-M-mode echocardiography (Fig. 2)
